ANCILLARY SERVICES  

1.  FORMS:

Depending on the nature of the form, it may take at least 48 hours to complete. In some cases, further testing may be required which would lengthen the wait time. In addition, some forms may be more appropriate for your primary care physician to complete. You will be informed if this is the case. There will be a minimum of $25 charge for completion of any forms.

2.  MEDICAL RECORDS:

Request of medical records will take up to 1 week. All records required more than 5 pages of Xerox or CD duplications which will require a processing fee minimum of $15.
